How to get the standard Kingpin Dethroned Accessory
The base version of the Kingpin Dethroned Accessory drops from defeating Kingpin himself, the hidden boss in Blood Hunt’s Extreme difficulty mode. You only need to beat him once to claim it, but reaching that fight takes real progression.
Extreme difficulty is not available from the start. You have to clear Normal and Hard stages first before Extreme unlocks. Once you’re in Extreme and meet the specific requirements to trigger Kingpin’s appearance, defeating him for the first time rewards the accessory automatically.

What triggers Kingpin’s appearance?
Kingpin is a hidden boss, meaning he doesn’t appear in every Extreme run. Shards drop randomly when playing Nightmare 20 or higher difficulty levels in Blood Hunt. A small number of shards can also be picked up through event achievements, specifically for beating certain Nightmare difficulty levels for the first time. However, the achievement shards alone won’t get you anywhere near 100. The majority must come from repeated Nightmare 20+ runs, and since the drop rate is random, how quickly you accumulate them varies.

Equipping either accessory works the same way as any other cosmetic in the game.
